...to my site containing all of the forms you will be required to submit to your clinical faculty person either during or at the completion of each of the clinical courses in the Family Nurse Practitioner sequence. I have also included the clinical practicum guidelines which are sent to preceptors and other clinical guidelines for your review. The materials are in .pdf format and thus need to be read and printed through Adobe Acrobat. If you do not have Adobe Acrobat Reader, click on the image to the left and you may download a free copy of the Reader. Then, simply click on the required form listed below and print it out for your use.

Forms to be used in clinical practicums include

of Patient Encounters
This form is used to submit a weekly and summative log of clients seen. - Culminating
Clinical Behaviors Checklist
This checklist is used to record a tally of the types of procedures and health problems the student experiences during the preceptorship. It is to be submitted at the end of the semester. - Visits Across the
Lifespan Tally Sheet
The Tally Sheet is a record of age groups and chief complaints seen by the student during the preceptorship. This summative record is to be submitted at the end of the semester. -
Clinical Site Visit and
Evaluation by Course Faculty
This form is used to provide instructor feedback following the clinical site visit during which the student will be observed conducting a primary care visit. - Preceptor
Evaluation of Student
Students are expected to request an evaluation from each preceptor for the semester. This form should be forwarded directly to Dr. Wilson for inclusion in the student's file.
Student Evaluation of
Clinical Practicum and Preceptor
Students are expected to submit an evaluation of each preceptor site they use during the semester. Clinical Preceptorship Summary
This form is used to maintain a running tally of clinical hours spent with each preceptor. This is due after the clinical hours required for the course are completed.
